Recall Information

In cooperation with the U.S. Consumer Product Safety Commission, Our company is conducting a voluntary recall of our pack and play mattress to address the risk of infant suffocation.

This recall involves Forias pack and play mattress ASIN B0BXLLSHJ8, sold online at Amazon.com from May 2023 to August 2023. The portable memory foam play yard mattress measures about 38 inches long and 26 inches wide. There are tags on the bottom of the mattress cover and a stitched tag with the brand name “Forias”.

How do you take advantage of the recall?

Immediately stop using the pack and play mattress.

To obtain a refund:

1. Share a screenshot of your Amazon order number and a photo of the date code on the mattress.

2. Take a photo - showing the destruction of the mattress, cut in half and unusable.

3. Email the photos of all three of the above to us at customer.care@forias.com

Where can I find the Date Code on the mattress?

The date code is sewn in the mattress cover and is formatted M, Y.

Note：
According to CPSC requirements, products eligible for recall must meet the above requirements for photos.
Model: FS-FCM Trifold Pack n Play Mattress is not included in the recall.
